Care Connection Team (CCTs)
The Acrefield Care Connection Team (CCTs) look after patients aged 65 years and over, who have been identified as needing case management because of their health and social care needs. Where appropriate, a care plan will be discussed and agreed with the patient on how best to manage their needs.
