Dear Patients, please note that we can only accept Non-NHS work requests in hard copy written format using our Non NHS form which must be signed and dated along with the accompanying Disclaimer statement on the reverse which must also be signed and dated – please note if you are submitting a request on behalf of someone else, the Disclaimer must be signed by the patient themselves. If the correct signatures are not provided, there will be a delay on processing the request. Unfortunately we are unable to accept Non NHS requests via email
